at address you have too much of a forward press with your hands. You do want this, but I would look down and want my hands over the ball at address with the 5 iron, as with most irons. Also check your grip, your left hand seems very strong. I would make it just a little weaker by turning it to your left.
your hands are too far forward, too much weight on the back foot, too wide of a stance, shoulders are way too unlevel. to fix it, shorten your stance width, put 60% of your weight on your front foot, let your hands stay relaxed at belt level, and finish with 90% of your weight forward

you started with a lot of tension on your back foot. stay more centered over the ball, it'll lead to better contact, and therefore better shots.

dont shift ur weight to the back. shift a little to the front leg. =)
