Ataru was able to keep himself semi-together through all the other "deaths," even Shinobu's. When Lum's turn came, he absolutely lost it. "gimme back my Lum!"
This episode shows that Ataru doesen't just care about Lum, it's a lot more than that.

the music bits and gun shots made the whole bit interesting-before the hospital door opened I knew Ataru would be hitting on the nurse.

They were way over the line before lying to Lum to get her to go along. LUM may love ataru but this AIN'T how to show it...

You could say that it was hard to tell because he could have been loosing it since it appeared his time was even closer at hand.
However, because he stayed with Lums body the whole night (at least I persume that's what happened) it appears that he did care for her the most.
