For the chorus its starts with the same bar chord on the fifth fret with the added 9th, then an open D chord, then a bar chord with the added 9th on the 9th fret, then the stretched major chord on the 4th fret. Pretty much its the same chords the whole song just switched around for the chorus.

It's in drop D tuning (if you want to get technical low-high) Db Ab Db Gb Bb Eb) The first chord is a bar chord on the 9th fret with an added 9th played with the pinky on the high D string 11th fret. Then it's an open D chord with the pinky sliding from the 11th fret on the high D string to the 12th fret. Then it's another bar chord on the 5th fret with an added 9th played with the pinky on the high D string 7th fret. Then there's a stretched major chord on the 4th fret with the pinky stretching
