2. I kept the paper with the pin number in my wallet all month. (I work from home; so usually -- we had house guests for 6 months that may have used the cash advance to pay their $400 share of rent -- there's no reason to suspect anybody outside of my home getting access to my wallet.) And the bank is used by a lot of people cause it's accessible.

53 literally sucks. So far as I'm concerned there was a fraudulent cash advance made on my account. 1. I got a cash advance early in May and, due to the limits on the account, would not have made a second request later in the month that would exceed the limits.
