At the start a guy has a shotgun then the old lady open the door for a dog the guy dies and you could get it at the start that simple

If I go there early in the game, before Cletus is there, three other characters appear: 2 guys and 1 woman. One has a machine gun, the other two have shotguns. If you walk in the door, they wipe you out.

Seriously? Whoa! Did they have an update or something? (even though I doubt it because the game is quite old)

Yeah, they're people you can talk to (after they finish trying to kill you), and take back to the safety room. Oh, and the woman has a pistol.
If you don't want to talk to them, you can take them out by standing at the door and being very patient shooting at them long-range. Fortunately, there is a long corridor nearby, full of zombie cops, all ready to drop their pistols...
