Explaining for those who haven't gotten this far. You just entered the virus into Big Shell, which corrupted any electrical device in the structure. With the Skull Suit and fake blood Raiden was sporting, he was getting effected as well, which messed with his perspective of real and fake. The Metal Gears have always been known for their mind-boggling death simulations. Any hardcore fans who have played mgs3 would know with the river scene.
