The guy is real but the insertion is not. Look at the jump cuts during insertion. They probably cut the tube.

It wasn’t live. That being said, we’ve often done training with cut tubes in CPR manikins or in real people and you’d be surprised at the suspicion of disbelief that the participants hold. They act as if it were very real.
