Monopolies only exist through the behest of government, Bell Telephone was a great example of this. Governments didn't like it when everyone was putting up unsightly telephone poles, so they stepped in and gave limited contracts to a chosen few. This is still the reason why America is trailing in internet bandwidth. Municipalities pick the provider they like and don't allow others to compete by putting in their own lines.
