I like it when games don't end immediately after killing the boss. The final rooms with the huge machine were a good idea.
This is, actually, a major similarity with Quake 2: Ground Zero, which was also developed by Rogue. It also ends when you destroy a huge machine :)

Actually your worst enemy on this map is the... random quakes (fitting).
You can easily get shoved into the lava losing lots of health and mobility vs the dragon's missiles.

When I try to choose episode 2 from the Introduction level (skipping episode 1), the game crashes for me. Is it intentional, made in order for the player to complete episode 1 first?

When I got to the end myself, I was still a keyboarder. Nevertheless, I could not defeat that dragon at all. Later, I lost the mission pack copies (they weren't mine so I had no CD either), so now it's the first time I see DoE finished.

The Grunts can be quite annoying and effective baddies. Too bad id Software did that strict distinction between military and medieval (as well as between knights and deathknights) when they made Quake.
