What a tutorial ?. You didnt even tell us what script should be used for the submit button. You just cut it from somewhere in your system and pasted it for yourself. And how are we to know what code is used to send the variables values to the target email ????

Hello i have the same problem i dont recive any e-mail ,everything else is perfectly working good job but give me an idea how to make it to recive the e-mail? Is there somthing missing in the Action or where?

For some reason, when I try to enter text, most of the buttons and none of the numbers work. Do you have any idea what might be causing this?

And to everyone complaining that you're not receiving emails, you need to be hosting an SMTP server, you can't just magically conjure-up an email without a database.
