I was faffing around with my yarn yesterday trying to work out how to do the German twisted cast-on with the yarn tensioned around the neck. I think this is it.

Note: My video shows how to set up to start purling on the purlside (so that you can join up to start knitting in the round from the purlside), as opposed to joining the round and purling on the knitside, which is what would happen if you just did this cast-on in the "normal" way.
Why would you use this cast-on? Well, firstly because you are wanting to knit "portuguese style", but mainly because it's a nice stretchy cast-on ideal for ribbing on sock cuffs, mitten cuffs, hat bands, sweater waistbands etc.
