This show borrows methods from the aerospace industry, as well as other industries where safety is critical. It then applies these methods to the process of taking patient safety to a higher level. It does this by using several hazard analysis tools and mitigation methods which all hospitals should use, or at least consider the possibility of using.
Thanks Dr. Raheja! Blaming nurses, docs, etc will NOT solve the problem of medical errors! Our culture is blame oriented and building trust will be important in applying 'fault-free analysis' successfully.
