This show borrows methods from the aerospace industry, as well as other industries where safety is critical. It then applies these methods to the process of taking patient safety to a higher level. It does this by using several hazard analysis tools and mitigation methods which all hospitals should use, or at least consider the possibility of using.

  • Thanks Dr. Raheja! Blaming nurses, docs, etc will NOT solve the problem of medical errors! Our culture is blame oriented and building trust will be important in applying 'fault-free analysis' successfully.

    Our work requires vigilant concentration & our work environments are filled w/ noise & constantly shifting priorities.

    Will interruptions, # alarms, pages, questions etc show up as contributing factors w/ Fault-Free Analysis? Beth Boynton, RN, MS
