You would need to slice the image into many smaller images and have a knowledge on html to do so.
It makes it difficult with paint to slice, but once you do you can move from there.
Just manuel coding, or if you have a program that will code with a good UI that would be great too. (ei. dreamweaver)

yes, as the title clearly states this is for the layout.
After you create a layout, you slice and code.
html is basic, I would recommend learning indepth css and then moving onto php.
