hows this version in english? what system are you playing on?

Sissy is only available on the AES/Console, and can be picked by holding all four buttons after you select any character.

Judging by the background animation, you only have the crappy tunes from Rage Of The Dragon on. Go to the options (in arcade mode) and set it to USA. This should enable the good music.
Very funny video by the way. To sum it up: play as the broken boss character.
