Based on the questions posted as:

There has been talk of making contact with this kid. I think that even with his obvious mental health issues he is a potential gold mine of information.

If I had an opportunity to talk with him (I am way too far away) these are some of the things I would want to ask him about:

Details and circumstances concerning his termination from staff at the Org.
Any paperwork? More information concerning the reasons they provided for his termination.
There are possible Americans with Disabilities Act violations, wrongful termination issues, and plenty of generally bad press for an organization that claims its the moral alternative to the evil psychiatric industry if they just tossed a mentally ill person out on the street when they were done with him.

How did he first become involved with Scientology? What was he seeking from them?
What promises did they make him? What did they say they could do for him?
We need to establish the nature of the relationship between him and the organization.

Details concerning how he went from just a member to a possible staff position.
Any paperwork or information concerning his performance. Evaluations? Why, if his mind was a liability was he promoted and hired to staff?

Attempt to obtain the identity of his father that he talks to in some of the videos. Is he also a member of scientology? Im going to bet that since this kid doesnt seem to have a lot of money that the cult milked his father with promises of helping his obviously disturbed son.

How much money and time did he donate to Scientology? How much money and time did his father donate? What, if anything, was he told he was going to receive for this money and time? Did they imply or promise that they would help or heal him in any way?

In one of his blog entries he mentions being severly in debt. How much of that debt was related to donations to Scientology? Was he still donating while he was in debt? How much was he donating at that time? How much did he donate a month on average?

The date of the fundraiser video. Any other fundraising information.
Remember they got Capone on taxes. Even though the church doesnt pay them there are strict laws regarding fundraising. You cant inflate numbers to raise funds, or provide the donators with false information. Im very curious as to whether or not the information on that slideshow presentation in the video matches their federal reporting.

What was he doing in that storage shed? What was he supposed to be doing? Was it an assignment? Who assigned him? What, if any, instructions or training was he given concerning the confidentiality of the sensitive materials stored in the shed? Was the shed normally secured? Was access to the shed normally limited? Could any one from the Org have gone in there and read those files?

What is his age? What was his age when he was drinking beers with Mr. Jim?
Where did that drinking take place? Who provided the alcohol?

Any more videos, photos, or documents?

Has he been in contact with anyone from the organization since they let him go?

Have they checked up on him in any way?

Has anyone from the organization contacted him concerning the uploading of those videos to Youtube?

Agreed. GET THAT FORM. Make them give you something in writing. As you said, they owe you that much.

They don't give forms out when they think there might be a legal liability-- i.e. they are hiding crimes.
