Can someone explain how the Old Kingdom works? What factors decide whether you end up there?

A lot of sick patients end up there - Earl Candleton when he's shot himself and is in a very deep coma, Peter ends up there many times, but then there are others - Stegman isn't ill, and yet for some reason his elevator is randomly hijacked and he is brought to the old kingdom, the headless dude is DEAD and yet in the old kingdom he's still alive - how does it work? Who goes there, and why?

@saturnino4 Because it's creepy? Because elevator 2 crops up a lot? Because they'd already done ice cream man? Mostly I think it's just for the creep factor, old uniforms are creepy.
