One of the 8 Dragon Priests, Nahkriin, in The Elder Scrolls V: Skyrim. Acquire it from the Dragon Priest Nahkriin Skuldafn during the main quest "The World Eater's Eyrie".

Note: You can not normally return to Skuldafn so you must obtain it then.

Mask effects:
+50 Magicka, and Destruction and Restoration spells cost 20% less.
