Toshio was murdered in the bath AFTER his mother was killed. Kayako was killed first because her husband thought she was cheating on him. So he killed her and then began to wonder if Toshio was really his son, so he killed Toshio. I've seen all the original Japanese Ju-On movies. The woman is Kayako Saeki, her son is Toshio Saeki and the man who killed them was Kayako's husband, Takeo.

I'm guessing their souls/minds or something like that got mixed because they died together?
Anyway, the kid didn't witness as much as the girl did, plus he didn't really understand.
The girl's grudge is way stronger because she actually saw the murders and understood what was happening.
