Fettel and his brother were part of Origin, along with Alma. Perseus was a project intended to train clone soldiers to take orders from Fettel. Icarus is the project responsible for making the ninjas.

No, it STARTS around the same time the first game ends, and both games feature a nuclear explosion. The nuke in the beginning of F.E.A.R. 2 is the same one seen at the end of the first game.
