I think he is referring to killing the part of himself that is addicted or causing him trouble. If you have ever had to do this you know that part of you can scream and resist being cut away. It can be very difficult to go through and you can not want to feel it. And want "one more medicated peaceful moment" that line makes me cry too.

(continued part 2) he is trying to sever any umbilical residue because he does not want to subject her to where he has fallen to. he feels it would kill her to know how he has fallen. he doesnt want to pull her down with him. it would seem obvious that addiction would be part of what his downward spiral consists of, because even in the midst of this realization he just wants one more medicated peaceful moment....he loved his mother very much. it would not be about wanting to kill her.

actually i think he is traumatized from what his mother went through and it was probably a major cause for the self medicating and the famous "one more medicated peaceful moment" because he does not want to feel the hostility left from the experience of childhood. but when he says he wants to cut away, sever the umbilical residue "keeping me from killing you" its not that he wants to kill the mother, he wants to keep from killing her. and pulling him down with him (continued)
