no the actor actually did douse herself in hot oil just as planned and she suffered hideous facial burns. Nothing went wrong, that's how it was ment to happen while filming.
Afterwards she was MENT to survive and have debilitating facial scars. But she ended up dying of infection in hospital, thus she was sued for breach of contract.

Hot oil is the only thing in a kitchen that would inflict those kind of injuries. Boiling water will only give you blisters. Still very painful, but it won't melt your skin off.
