Just play a lot, you will get that anyway by aiming for S ranks and beating all game modes.

A fast way I figured is to play mission 7 and keep breaking the red orbs crystal at the beginning then retry and keep doing that, lastly save your game. Didn't really understand your third question.

@lan21312 Heaven and Hell is whereby EVERYTHING dies in one hit including urself while Hell and Hell is a mode whereby ONLY U ( nero/dante) DIES in one hit =D
