Here's the situation, IMHO: Dean was competing with Washington insiders like John Kerry, John Edwards, etc. and he made them nervous when he got so popular, so quickly. The so-called scream wasn't a real controversy. It was something that his opponents methodically blew out of proportion. They figured by labeling him unstable and a little crazy, it would cause just enough people to think twice about voting for him. Sounds silly, but apparently, it worked.

I mean, it's not like he is dangling a baby over a a balcony, or having a public meltdown, or puking under a diplomats dinner table.
He got a little high pitched. I just truly don't see the controversy.
I'm really trying to put myself in another viewpoint's shoes, and I just just can't see it.
