You're off with the F# and G transitions, you're mixing them up. Also, the chorus goes E/G#/F# BACK to G# and the breakdown is actually C# to B, F#/G/G#/E.

the bass i have now is a 5 string...5 strings are much better then 4 strings in my opinion.
