This is probably the most nonchalant way anyone has announced they have a terminal illness... Ever.

How dare you? Chris-R's performance was so good, he was probably a real drug dealer who came onto the set to collect Denny's actor's debt. The others were supposed to be the first people to make it onto the rooftop, when that failed, they decided to ad-lib the whole scene since whatever they made up would be a billion times better than what was in the script.

This is the only thing in The Room that could be interpreted as something that makes sense, here's why:
Claudette is clearly a manipulative drama queen, so Lisa assumes she's lying as she has done so about serious issues before.
Lisa also shares those traits, but is much less skilled about it than her mother. Claudette is too good a liar to be deceived, so she realizes that Lisa's story is false because Johnny hasn't drunk in a long time, which makes the story implausible.

If I hadn't seen the rest of the movie, I would have guessed that the mother had pretended to be dying many times before in order to gain her daughter's sympathy. Having seen it in its entirety, I know that this would be giving too much credit to Wiseau's trademark "incoherent non sequitur" brand of writing.

This is probably the breast cancer line I've seen in a movie.

He only changed his stance because it attained Midnight Movie cult status as a "hilariously awful" film. He then started saying "Haha! Joke's on you! I made this as a black comedy". It was his way of saving face. Every person who has ever been interviewed that worked on the movie said he shot this as a straight serious film, never, ever, did he say this was a black comedy.

Most likely not. But Wiseau has zigzagged on his stance towards his film. He initially characterized "The Room" as a serious drama but retrospectively changed the characterization to that of a black comedy.
But that's part of the fun of "The Room", trying to read things into it that were probably never there to begin with. ;-)

I seriously doubt Tom Wiseau even considered that while making this scene.
