Everyone take a look at the REFERENDUM as you read this so that it's absolutely clear. Please be sure to read this from top to bottom and understand it clearly. Remember only San Diego City resident voters only. Not El Cajon, not Lemon Grove, etc, though they can definitely donate.

1. Ask the 3 qualifying questions before having the voter sign the Referendum petition
• Are you a registered voter in the CITY of SAN DIEGO? (Address must have for city: SAN DIEGO)
The signature will be disqualified if the voter is not a resident of SAN DIEGO
• When was the last time you moved?
You must re-register to make certain that the voter registration address and signature matches the Referendum address and signature.
No abbreviations. The space is for the address is small, so ask them to write small so that it fits.
• When was the last time you received ballot voter information?
• If not recently, they you must re-register to ensure that there correct address is on file.
No abbreviations. The space is for the address is small, so ask them to write small so that it fits.
Example:
CORRECT
1234 Main Street
San Diego 92113

INCORRECT
1234 Main St
SD, 92113

2. Address: Again No abbreviations. Make sure to fill out the county as well.

3. Date of Birth and State or Country of Birth: These must filled out it will not be valid

4. CA ID/DL #: This is the preferred answer however if the voter doesn't have one they must write last 4 digits SSN

5. Political Party: Must select an answer: Select a part or mark no I do not want to register with a political party.

6. To receive a vote-by mail ballot in all elections: make sure to have voter's initials

7. Are you a US Citizen? Answer must be YES

8. Will you be 18 or older by the next election? Answer must be YES

9. Signature: The voters signature needs to match the way they signed their registration form (if they don't recall how their signature looked then re-register them)

10. Date: Make sure that they put the same date as the petition signature

11. Print Name: Their name here should match their name as registered. If James Brown registered as James Joseph Brown then he should write "James Joseph Brown". If he registered as "James J. Brown" then he should print "James J. Brown" in our referendum.

12. Date: Make sure they put the month/date/year. So 4/29/11 or 4-29-11. DO NOT PUT 4-29 or 4/29 without the year and think it will be ok.

13. The "NEW REGISTRATION" box to the far left of each signature column...this box is where we put the serial number if the signer also completed a Registration Form. Write the first 4 digits and underline it underneath it write the 6 digits. You will find the number under the voter's signature and at the bottom right corner and just under the signature box, it is 8 digits.
It is important to make a copy of the voter's registration form and attach it to the Referendum packet that the voter signed.

Example:
37 BP
--------
123456

If someone makes a mistake you will need to CROSS OUT THE LINE AND HAVE THEM START AGAIN ON THE NEXT LINE. Double check everything.
