This one can be a bit tough. You are limited to two minute runs in Trauma and you have to get at least a 4x combo to get this. However... you only get combos for Pain hits and character hits in Trauma. That's what makes it tough.

So... you can easily get two Pain hits by hitting the camera crane and the half-pipe, then a character hit by hitting the alien. On this run I got lucky because the alien flew off, hit another alien, and then a pedestrian ran into them, giving me a 5x combo! I then proceeded to crush the 200k mark, ending up with 321K.
