I believe she isn't dead when Harry meets her, but the sole nurse remaining to become "transformed". Heather states that she is grateful for the care Lisa took of her, so Lisa was only an innocent bystander devoured by Otherside, much like Harry.

The good ending is canon, not the good+. The algawhacthamacallit is collected in the hospital, and Harry gives it to heather, the necklace with the red orb at the end of 3

The person in the wheelchair is Alessa, her physical body. The Alessa standing next to her is Cheryl/Alessa, the spiritual projection.

Lisa is dead (died of an overdose). When she says she's the "same as them", she means she's manifested like the monsters are.
