My finger generally slides off the string without catching while trying pull offs. Should I use my nail or try to get my finger behind the string a little more? I always wash my hands before playing maybe they are too smooth.

Distortion, if you have it its easy to do these, if you don't have distortion its harder on an electric than on acoustic.
Really depends most on your amp settings.

does anyone know how to stop the string from just stopping when i put my finger down to do a hammer-on or take it off with a pull-off?
it might just be that i'm not doing it fast enough, but I often lose some or all of the volume on the 2nd note :(

DAMNIT! The 5 is on top of the 4 in time with each other! Piece of shit keeps moving the 5 to the left of the margin! Sorry!

The 5 and 4 are in time together. Since your striking the 6th fret and pulling off to the 4th fret with only one string then what do you do about the other string on the 5th fret? Do you hit the B string 5th fret as soon as you pull off the 6th fret to the 4th fret on the G string? Does anyone understand what the hell I'm trying to ask or say?

I have no clue how to play that.. I (very much) doubt that even sounds right. Definitely unconventional way of playing.. Sorry, I have no clue.

You should be 'hammering-on' the higher note with enough force that it makes a sound on its own without picking the string. That's a lot of force for one finger, but as you get used to it, it gets easier. Having said that, if you have tons of distortion on your sound, it's a lot easier, so start with lots and lots of distortion.

I have only a single pick up on my guitar so it dies really quick is it my guitar or my amp?

It's probably the guitar. Single coils usually have lower output; generally, humbuckers are easier to work with for hammer-ons and pull-offs.

My hammer and pulls offs sound very weak on clean its frustrating

Start with lots of distortion - it'll help you how to get the basics done and you can slowly lower the distortion level. However, your clean hammer/pull-offs will almost always be weaker than on distortion so that's normal.
