I believe the Stay Puft Marshmallow Man was just a man-in-suit effect, so someone probably just screwed up one of the costumes that day.
I can't believe that Mad Max thing made it to the screen, and for that matter that I've never noticed it. Wow.

Great stuff, Adam. I've heard about the Storm Trooper hitting his head, but could never find it in the movie. Thanks for pointing it out.
