Naomi survives in the true ending. And this is the bad ending for when you fail to do something, causing Naomi to "suicide" right after Seiko's death.

As for how to avoid it, read this video's description. You have to make Seiko lose her charm.

I just got this bad end, I immediatly deleted the game and save file, I'm never going to play again.... why struggle so much with a game that isnt even fun.
