Mayo Transitional Care

Transitional Care provides a step between hospital and home for patients who continue to need daily skilled care by and nurse and/or therapist. Patients who benefit from this type of care include those that are older, those suffering from chronic illnesses or those requiring daily therapy following an accident or injury.

As part of Luther Midelfort's Continuum of Care Program, Transitional Care is designed to provide rehabilitative care for these patients in a specialized family-centered facility to help them reach a higher level of wellness. Transitional Care assists them in improving their physical, emotional and social functioning needs to ensure a safer and smoother transition home to resume care on their own.
