In the beginning, the capo is not actually on any fret. Sandra has it sitting on the "0 fret" (the bit where the strings join onto the nut and opposite the fretboard side). Its only there so that Sandra has quick access to it for the key change.

@guitarholic999 Soundwise, there's no effect but the song changes key when the capo moves to the 1st fret and having the capo there beforehand makes the shift easier.
Some people, however, do have the "0 fret" muted (Antoine Dufour's handkerchief for instance) because it eliminates the slight ringing sound it creates.
