Koto is not a harp, but rather a zither. It sounds more like a harp but it is not one.

I don't think so - I suspect arthritis. Her hand is wrapped on the last three fingers. The picks are on thumb, index, and middle fingers. The Tsume (picks) are from the Ikuta school of playing (most Koto players use this type of Tsume). The right hand is not what has the problems it is the left that requires more strength.
