I think it's more likely that the guitar forgot what notes it should produce than Petrucci having played them wrong.

What happened is that John began overloading himself with music power, and in order to save everyone's life he had to counteract the awesome factor with a slight essence of plain old good factor. The ensuing combination of the two factors created a paradox since, even though John can access plain old good, he is in fact incapable of playing plain old good, and as such it caused a rift in the music-time continuum, perceived as, in our case, a "mistake".
