It surprises me that the authorities gives a clean tank certificate for a tank that is not clean... I seriously hope it wasn't out of laziness.
Sweden's Salén shipping lost a VLCC (T/T Seven Skies) while welding on a full cargo tank. It was before the inert gas systems and the dangers of tank explosion were not known.

it was back in 1997 and 2 of heavily wounded firemen died next day.The master of that tanker is a DPA at my current company.He told us everything as a lesson.3 fitters died also in fuel tanks while working,that was the first spark.The firemen families lawsuit still in progress because at back then the mayor of İstanbul is now our prime minister....

And why were they working in the tanks? Tank cleaning (no, fitters don't do tank cleaning)? I seriously hope they weren't wealding.
Had they no system of hot work permits? This leads to a lot of confusion for me.
