I would give instructions here, but there really aren't any. Just take your app, package it, and install it on your phone like any other app ("palm-install com.yourname.yourapp_1.0.0.ipk" at the command line). If you have the new Maps app, it'll work. If you don't have it then it won't work.
I forgot to show you on the video for proof, but for reference this is an original Palm Pre running webOS 1.4.5 - no doctors or OS modifications. Just OTA updates from Sprint.
