I would suggest the ART usb dual pre on Amazon $79. I bought mine for $55 at guitar center and use it every time I shoot video. It has phantom power, headphone monitoring and 2 XLR inputs and 1/4 inch outputs. Make sure you use quality cables (I have monster).

Rolls makes a 3 channel model MX54s ProMix Plus
XLR inputs x 3, phantom power, low cut, pan controls on each channel, headphone out, balance stereo output (XLR x 2) with adjustable trim on the outputs, not very big, and runs on 2 9V batteries.
They have them on Amazon for just under $130

To Clarify, I have mics... I need a way to mix down two into a camera, that supplies phantom power, lets me monitor my levels, and is belt mountable.

Ive got a Rode VideoMic Pro set up, It works great, you should deffinatelyy consider it :)
