I have the same problem with the driver. I think its mainly because you have too much of your left hand palm on the club. Grab the club with your right hand at the bottom of the grip on your club. Then start back on your swing, and when you get half way, place your left hand. Since your hand is hinged, your palm will not make contact with the club. It may seem uncomfortable, but practice it. Other than that you have a pretty nice swing.
Im a 6--8 handicap and notice your right hand is turned a little too far to the left-- too much on top of the club. Your hands look like they;re fighting eachother a little. Turn your right hand to the right some to match the palm of your left hand. Palms should face eachother with the back of the left hand and the palm of the right hand facing the target. This should/might help that slice/fade.
also when looking at the side on view, the ball is very far forward in your stance which is causing you to have your shoulders slightly open at address. This will mean you are more likely to be attacking the ball from out to in, as opposed to slightly from the inside. try bringing the ball back an inch or two in your stance, as this could make a massive difference.
you come over the top a little in your irons, but it is much more noticeable in your hybrid and your driver. try dropping your hands onto the correct plain at the top, and start with your torso when unwinding in the downswing instead of your arms. also your posture looks a little upright and you look a little tense across your shoulders and in your arms. also if you maybe swing a little shorter you will gain more control over the clubface. very good tempo though.
If your slicing/fading off the tee you might attribute that to either casting the club when you start your downswing, causing you to cut across the ball, or you right hand might be too far on top of the grip and crossing your left hand, which might also cause a slight fade or slice, but thats just a 14 year olds opinion
Don't listen to the tempo and swing length critiques. Your tempo is outstanding, and the width in your swing is ideal.
I would simply suggest allowing your hands to fall a little closer to your body at address with the driver. It might help you take the club back on a bit of a better line, and eliminate that slight over-the-line motion on the way down.
I wouldnt change much though, your swing is excellent.
