Okay, that's a reasonable question. Go into your 1.00g folder and place byte 224/E0 at address 520148/7EFD4 in TRAINER.LEV and it'll be a bit to the right of the starting position. Or you could place it elsewhere in some other level, of course, that's just arbitrary. The next three bytes represent its parameters if you want to try to fool with those.
I'm going to say... no, there isn't. If you don't understand hex editing, you're not going to be able to do anything, as there are currently no tools for editing 1.00g files. But you're welcome to download some free hex editor - I use XVI32 - and look through the help files until you get it.
No, they don't. This was the only event with any kind of surprise effect - everything else was either not-yet-programmed, functional, and/or already visible in the game levels.
