During those "bug" exercises do you keep your lower back arched or flat on the ground? And you are supposed to flex you abs a lot?

The back has to remain flat. As legs and arms extend further the back will try and go into extension or arch. You must then contract the abs to counteract that. IF the person can't maintain their back flat then bring the arms and legs further back to the starting position. Only allow them to extend their limbs as far as they can maintain a flat back.
Hope that helps
