@charlesbrownish - The head nun at the orphanage where he was raised invented a story of how his heart became sick, so he was given a baboon's heart after his missionary parents were killed in Africa (none of which was true). She wanted him to feel special because he was a sick little boy no one wanted. Later Adam was afraid a transplant would mean losing his "baboon heart."
FYI -- the film's original title was "Baboon Heart" but it was changed to appeal to a wider audience.

It was as if he knew he was going to die, when she came to pick him up for the hockey game. And he wanted her to find them when she was ready to listen to the records. He knew her so well and loved her deeply.

@6677825kli He had a weak heart and needed a transplant. He didn't want one because he was scared that it would change his feelings... she tries to make him understand, but as he says so eloquently: Then why does it hurt here when you're not with me?
