Carlos de Seixas was a baroque Portuguese composer. Only few pieces survived.

What is this piece called? Who's the composer?

It is in the title the composer is De Seixas (pronounce as Seisjas) a Portuguese composer and the title was Sonata 65. Unfortunately lots of his compositions got lost.
