Alex, please see your local optometrist or eye doctor, where you would go for glasses, and they will refer you to a eye surgeon. Then their billing department can quote you a price.
Most insurances will cover it. If you are not from America, you could get it free, if you live in another country. But in america they make you pay part of it usually.
Mine cost 2800 dollars, and I had to pay 500 as co pay. I have Kansas state insurance-blue cross. Costs more with two eyes, double probably.
