First you need to get a full bar (Secound bar, where is next to your health bar in the top left of your screen.You can full it, attacking several enemies) Aim an enemy with the Right mouse button. With the enemy aimed get near to it, PRES AND HOLD Q Button, release right mouse button and move your mouse quickly to the aimed area in the enemy's head. When its all red, Push the left mouse button.
