Fuel cap loose or falls out of fuel port. Fuel spills on ground and onto firefighter's gear as he walks up to prop. Sparks from metal being cut by power saw ignite fuel.
Even simple training can go very wrong very fast. Thank god for turnout gear. With no gear that results of a gasoline spill like that could have been much more painful and life changing.
