------------------------------ ------------------------------ It's not real, it's a delusion he's having. In reality he's still lieing there in his bed after being operated for him being shot in the head again. In the delusion Johan starts talking in his mindtwisting Johan way and it frightens him. Not long after that Johan disappears from the hospital just like when he was a kid -

@Mohammed7756 It's about a surgeon who is brilliant, has everything you'd ever like untill one night a boy who got shot in the head and the mayor come in the hospital and he has to choose who to save. He saves the boy and the mayor dies and he gets demoted. His fiancee leaves him and the same night all the people who demoted him and the president of the hospital are found dead and the surgeon gets blamed for it. He has to flee and try to proove his innosence, that's pretty much the big storyline
