That's just a huge fucking needle, the dermal punch is much faster and most likely has better results than the needle.
It's not a blowout, a blowout implies the fistula was torn out. I think his ear is just traumatized from the force the lady had to use on the needle. A dermal/biopsy punch would have been better (in my opinion).

If I'm Not mistaken isn't that a Derma-punch needle? Hows that "friggin' Rad" piercing? did it blowout? agrees w/ forestleger69 i see a chunk o' skin there too.
