when you don't start in Madagascar it's extremely hard to infect due to the only way in is the ship yards...

I infected all of the countries and then a few suddenly stopped dying. Like, the remaining infected became healthy and I didn't kill everyone. I don't get why that happened. Can someone explain that to me?

You have to get your infection's level higher, and get the right immunities and tranferrabilaties.
