A simple CHECKLIST would have prevented this.
All pilots need to understand the importance of using checklists.

I teach Pg for a living, have done for 22 years, This is one of the greatest dangers, the Human Brain, I teach pre flight checks to be vital and your life depends on them from Day 1. However, even though we drum this in in school it does not always stick. I have stood in front of pilots " have you done your checks" i say. yes they say. as they go to launch i shout stop! Look down. Now do your checks don't just say them in your mind! The pilot must have been stressed at T/O to not feel no straps

@seekndestroy454 no, you can't strap in while flying. It's like pulling yourself out of a swamp by the hair. Once I had set my leg straps to strong for my new suit. I was unable to get into the sitting position. The straps had cut off all the the blood in my legs and I was fallen half passed out before reaching the ground.

why couldnt he control the glider? would pulling on the breaks mean that he'd push himself out of the harness?

accidents happen. we must re-learn from them. he could have strapped in while he was flying. This is clearly stated in the manuals and emergency procedures learned in our training. If he froze, he needs to consider another form of recreation.

Why didn't the pilot just do up his straps in the air? Why did he not fly the glider to a clear landing? His first mistake was not doing up his straps, his second mistake was not doing his pre-launch check. But his third mistake was the worst. He flew for how long without simply buckling in.

If he could have done up his straps in flight he would have, Fly to a clear landing? Very little control if any...try it yourself one day and see how well you do when you think youre about to die and can do nothing but hang on. By the time the pilot "crash" landed he had almost completely slipped out of the harness and had no feeling in either arm from lack of blood circulation.
